GUIDELINES FOR SELECTING A CAREGIVER
1. IS YOUR AGENCY LICENSED IN THE STATE OF ILLINOIS? ARE YOUR CAREGIVERS BONDED? Answer: Yes. we are licensed by the Illinois Department of Public Health. All employees are bonded. |
2. WHAT KIND OF BACKGROUND CHECKS DO YOU RUN ON YOUR CAREGIVERS? Answer: Criminal Background checks, as well as, professional and personal references. |
3. DO YOUR CAREGIVERS SPEAK ENGLISH? Answer: All caregivers must be able to read, write and communicate in English. |
4. WHO IS RESPONSIBLE FOR CAREGIVER’S PAYROLL AND SOCIAL SECURITY TAXES? Answer: We assume all responsibilities for payroll requirements. |
5. ARE CAREGIVERS COVERED BY WORKER’S COMPENSATION INSURANCE IN CASE THEY ARE INJURED IN MY HOME? Answer: All caregivers are covered under our workers compensation policy. |
6. WHAT ARE YOUR HOURLY (OR LIVE-IN) RATES:

7. HOW ARE THE BILLING AND PAYMENT OF SERVICES HANDLED? Answer: Invoices are mailed to clients weekly. Payment Methods: Cash, checks, credit cards and insurance. |
8. IF WE CHOOSE TO, CAN WE INTERVIEW POTENTIAL CAREGIVERS AND IS THERE A CHARGE? Answer: Interviewing a caregiver is free of charge. Any additional interview is billed at our hourly rate for a minimum of two hours. |

10. IS THERE SOMEONE AT THE AGENCY TO ANSWER MY QUESTIONS OR CONCERNS AT ALL TIMES? Answer: Someone is available to answer questions 24 hours a day, 365 days a year. Our operator is able to address staffing needs immediately. |
11. ARE THE CAREGIVERS SUPERVISED? IF YES, IS THERE AN ADDITIONAL CHARGE FOR THIS? Answer: One of our nurses or staffing coordinators will perform both surprise and scheduled supervisory visits. |
12. WHAT HAPPENS IF MY CAREGIVER TAKES TIME OFF? Answer: Gentle Home Services is responsible for all staffing needs. We will assure a temporary replacement takes over. |
